Who We Are
Healthy Together Cleveland is a community coalition dedicated to improving the health of Cleveland County. Our coalition was selected to participate in the Healthy People, Healthy Carolinas initiative. This initiative is funded by The Duke Endowment and allows the coalition and its partners to identify the greatest health needs in Cleveland County, collect input on focus areas, create action plans, and improve the health of the community. The grant supports efforts on healthy eating, active living, and chronic disease prevention. Healthy Together Cleveland will use a collective impact framework, which brings people together in a structured way to achieve social change.

Partnership
No one or two organizations can accomplish this mission alone. Healthy Together Cleveland brings community members and representatives from local organizations together to identify and discuss what the greatest community health needs are, build work groups to create action plans, and take action to improve the health of the community.
Coalition Work Groups
Healthy Together Cleveland benefits from the active participation of members and partner organizations. Healthy Together Cleveland has established work groups to address health concerns. Work groups are responsible for establishing action plans and use a community-based approach to addressing physical activity, nutrition and obesity problems, factors that contribute to the development and complications associated with many chronic illnesses.
